Pain Management

Chronic pain can significantly impact the quality of life for patients and their families. Our pain management program is designed as a team approach.  Our team includes patients, physicians, caregivers, skilled nursing professionals and other therapeutic disciplines as required. Together, we develop a holistic treatment plan based on each patient’s needs. Using the individualize plan to teach the patient and their family members skills needed to help them continue to control pain after the home health care has ended.
